The long-term care sector is entering its most dangerous stretch of water in a decade. Medicare rates are rising on paper. Underneath, the Medicaid financing that pays for most long-term care is being restructured, and the rocks are in sight.

On October 1, 2026, the FY 2027 SNF final rule raised Medicare Part A rates by 2.4%. It also signaled scrutiny of PDPM case-mix coding, shortened quality-reporting deadlines, and moved toward all-payer MDS reporting. Home health agencies face a proposed temporary behavioral cut. Starting in 2027, Medicaid adds work requirements, six-month redeterminations, and shorter retroactive coverage. Provider-tax phase-downs and caps on state-directed payments follow, putting pressure on state budgets and on the optional HCBS benefits that many home care providers depend on.

In “Rocky Shoals,” Irving Stackpole and John Sheridan of Stackpole & Associates explain what is changing, when, and what it means for revenue, census, workforce, and capital. You will leave with:

  • A clear 2026–2032 timeline of Medicare and Medicaid changes affecting LTC
  • A framework to stress-test your payer mix and state exposure
  • Early warning signs that your market, or a competitor, is heading for trouble
  • Strategic options: service-line repositioning, partnerships, Medicare Advantage contracting, and diversification into private-pay and wellness models

This session is for executives and boards of SNFs, home health and home care agencies, HCBS providers, and senior living operators who need to make decisions now, not after the rules are final.

Seats are limited. Register today and bring your toughest questions.
